Andy Cohen has been at the center of some memorably chaotic moments in the clubhouse of his Bravo show Watch What Happens Live, but the host may wish he could forget the interview that led to what he calls his biggest on-air blunder: a sit-down with Oprah Winfrey.

The 56-year-old recently recounted his iconic faux pas during a conversation with Today show anchors Savannah Guthrie, Carson Daly, and Craig Melvin.

"You know I lean into a blooper," Cohen told the broadcast TV cohosts before recalling his last-second flail with Winfrey. "The show was about to end and I, idiot that I am, turned to her and asked, 'Have you ever swam in the lady pond?'"

Melvin immediately began laughing, joking that he should have asked Winfrey that, while Daly was insistent that Cohen reveal what the media mogul's answer was.

"She said no, and then Gayle [King] said that she didn't get the question," Cohen revealed.

Viewers with keen memories will remember the interview from Winfrey's 2013 appearance on WWHL, during which the award-winning journalist definitely seemed to understand the question when she responded to Cohen with a direct "No, no I have not. Thank you."

Last year, while celebrating his show's 15th anniversary with Entertainment Tonight, Cohen said that despite the invasive question, Winfrey was "super cool" and "it was not awkward" after the cameras stopped rolling.

"I was so grateful that Oprah did the show, and I still am," Cohen added. "That remains, I think, my favorite episode."

Daly, Guthrie, and Melvin also shared some of their on-air snafus, with varying levels of severity.

The former TRL host recalled how, during his early time at Today, working with a teleprompter and a big name like Leonardo DiCaprio tripped him up.

Guthrie recounted her 2012 interview with Khloé Kardashian, wherein she asked the reality star about speculation that she wasn't really related to the late Robert Kardashian.

And Melvin looked back on a gaffe that "almost got us sued."
